Here’s how the program works:

• Print out this form, and write your name on the $50 referral certificate below.
• Give it to a friend, neighbor, co-worker, or family member who has never been to our practice.
• This certificate will give them a $50 discount on their first appointment.
• When they redeem their certificate, we’ll thank you with a $50 credit on your account.
• There is no limit to the number of $50 credits you can earn.
